Information from the Nurse

Hours of service:

  • Monday through Friday 8:00 AM to 3:45 PM.
  • The RN., Darae Longhofer and/or her health assistant, is available in the clinic daily.

Services provided:

  • Immunization records maintained up to date by Texas State Law. Students must be current on their immunizations upon entering school.  The exceptions are provisionally enrolled students.  Students may be provisionally enrolled for 30 days upon awaiting transfer of medical records if transferring from another Texas school, a military family, or homeless.  Also if students are conscientiously exempt from immunizations, the form from the state must be current and presented upon entering school. Medical exemptions…the forms must also be current and presented upon entering school.
  • Vision, hearing, and scoliosis screenings as mandated by Texas State Law and necessary referrals.

Medications may be given up to 5 days with a written parentrequest containing specific information (ie: student's name, name ofmedication, dosage, time to be given, and how often may be repeated) and in theoriginal container. Medications given for longer than 5 days must have adoctor's note and be in the original container labeled as the doctorprescribed.
